    I. Introduction to Porcelain Veneers

    1. Advantages of Veneers:
      Veneers are partial crowns covering the labial side of teeth. Compared to crowns, veneers offer numerous advantages that often surpass those of crowns in most cases.

      1.1 Color and Aesthetics:
      Veneers achieve exceptional cosmetic results, often surpassing crowns in terms of color, shape, surface texture, and individual characteristics reflected through internal and external coloring. Special cements are used to alter tooth color and achieve perfect visual effects, resulting in restorations that are virtually undetectable.

      1.3 Functionality:
      In most cases, veneers’ limited application to the labial surfaces and incisal edges of teeth avoids postoperative functional complications. They are particularly advantageous for patients with deep bites, limited lingual space, or lower anterior teeth unsuitable for traditional crown therapy.

      1.4 Strength:
      New-generation dentin bonding agents and cements have expanded the indications for veneers. Edge placement is no longer strictly limited to enamel. Acid etching, silane coupling agent treatment, and new bonding materials have revolutionized bonding strength, achieving values comparable to those of resin-to-etched enamel systems.

      The veneer’s similar coefficient of thermal expansion to natural teeth enhances its durability. When bonded, veneers exhibit high tensile and flexural strengths, preventing fragility.

      1.5 Periodontal Tissue:
      Unlike crowns, veneer edges do not need to be placed subgingivally, preserving better gingival health. Edges can be placed at or slightly below the gingival margin, sealed with a soluble resin cement. However, polishing veneer edges is challenging and requires meticulous work.

    2. Disadvantages of Veneers:

      2.1 Irreversibility:
      Tooth reduction is irreversible. Veneers cannot be tried in or temporarily cemented. Once placed and bonded, they cannot be modified but can be removed (by grinding) and replaced with new veneers or crowns.

      2.2 Cost:
      Veneer fabrication and placement require high dental technical expertise. Adjusting color, shape, surface texture, and individual characteristics within a small area poses significant challenges for ceramists, resulting in considerable costs.

    3. Indications and Contraindications:

      3.1 Special Indications:

      • Fractured teeth in adolescents
      • Severely discolored single non-vital teeth
      • Large labial erosive defects and large cervical restorations in anterior teeth
      • Severe discoloration resistant to bleaching or minimal reduction techniques
      • Extensive enamel defects
      • High onlays for anterior and posterior teeth with erosive defects on occlusal surfaces and incisal edges due to acid erosion (e.g., bulimia, anorexia nervosa)
      • Deep bites with or without occlusal contact
      • Replacement of crowns in lower anterior teeth
      • Correction of minor malalignments

      3.2 Contraindications:

      • Severe bruxism
      • Extensive composite restorations and significant destruction in anterior teeth
      • High caries activity and lack of dental knowledge
      • Poor oral hygiene

    II. Corresponding Nursing Measures

    1. Adhesive Selection:
      Use Vivadent’s Variolink II dual-cure resin cement for its excellent bonding and physical properties.
    2. Surface Preparation:
      Treat the tissue surface of the veneer and the labial surface of the tooth. The veneer is roughened and steam-degreased in the laboratory before being sent to the clinic for acid etching by nurses. Use 5% hydrofluoric acid to etch the veneer’s tissue surface for 1 minute (avoiding the labial surface to preserve gloss), rinse thoroughly for 15 seconds, dry, apply Monobond-S silane coupling agent for 1 minute, then Heliobond light-cure adhesive, blowing evenly with a three-way syringe. This completes the physico-chemical dual treatment.

      The tooth’s labial surface is etched with 30% phosphoric acid for 1 minute, rinsed, dried, and coated with dentin bonding agents (Syntac Primer for 15 seconds, Syntac Adhesive for 10 seconds, both dried, followed by Heliobond light-cure adhesive). The combined use of enamel and dentin bonding agents enhances bonding reliability.

    3. Complete Bonding Technique:
      Nurses dispense an appropriate amount of adhesive onto a paper plate and mix with a plastic spatula for 10 seconds, avoiding bubbles. Place the veneer vertically on the tooth surface, accurately positioning it within 4-4.5 minutes at room temperature. Apply gradual pressure and hold for a few seconds. Remove excess Variolink II adhesive with a brush, curing selected surface areas for 10-20 seconds to stabilize the restoration. Do not cure any interproximal or gingival margins at this stage.

      After positioning, remove excess material with a scalpel or suitable tool, ensuring Variolink II does not spread beyond the margin.

    4. Finishing and Polishing:

      • Use a fine diamond bur (grain size < 25) and an elastic abrasive wheel to remove excess polymer.
      • Apply finishing and polishing strips to interproximal surfaces.
      • Check occlusion and make necessary adjustments.
      • Polish the restoration’s edges with silicone polishers (e.g., Polish-F/-P, Astropol) or polishing disks.
    5. Improving Oral Environment During Procedure:
      Moisture control is crucial for bonding. Gingival retraction aids in effective moisture exclusion.

    III. Precautions

    1. Strict Procedure Adherence:
      The bonding process involves multiple steps, reagents, and small instruments. Nurses should follow the “three checks and seven verifications” principle, verifying patient names, tooth positions, and adhesive ratios. Accurately time each liquid treatment, using different colors and symbols to label brushes, mixing wells, and reagents to avoid confusion.
    2. Careful Trial Blending:
      Select and blend trial shades per doctor’s instructions, testing them in the patient’s mouth. After doctor and patient approval, record the final shade and blending ratio in the patient’s records. Strictly adhere to these ratios during the final bonding.
    3. Timely Oral Cleanup:
      Once the veneer is accurately positioned, allow excess adhesive to partially set, facilitating easy removal with a scaler. Cure the veneer preliminarily in the mesial and distal aspects. Remove the gingival retraction cord, taking out excess adhesive at the margins. Thoroughly remove adhesive residue and complete the curing process with 40-second exposures at the gingival margin, mesial, and distal aspects.
    4. Safety Precautions:
      Trial shades can be irritating; shorten the trial process and thoroughly rinse the tooth surface to reduce patient discomfort. Hydrofluoric acid used for veneer etching is strong; avoid contact with skin, clothing, and immediately neutralize with powder after use. During light curing, advise patients to close their eyes, and wear protective eyewear to prevent light damage.
    5. Patient Education:
      Instruct patients not to speak or swallow during bonding to avoid saliva contamination. After veneer placement, advise patients to maintain oral hygiene, avoid hard foods, and schedule regular check-ups.

      Veneers (porcelain veneers) have multiple advantages in dental cosmetic restoration, including improving the color, shape, and surface texture of teeth. Through internal and external coloring and the application of special adhesives, veneers can achieve a natural and beautiful restoration effect, making the restored teeth almost indistinguishable from natural ones. These technical means not only enhance the appearance of teeth but also strengthen the overall aesthetic effect.
